Briefing note prepared by Zubeda Seedat for Sir Nigel Crisp in response to a request for a further meeting. The note sets out background to Lord Jenkins request, the attempt by Department of Health to source relevant papers. The note states that 1. Our understanding is that during the HIV litigation in the 1990s many papers from that period were recalled. We understand that papers were not adequately archived and were unfortunately destroyed. 2. In addition we have established that many other important documents, mostly papers and minutes of the Advisory Committee on Virological Safety of Blood were destroyed in the 1990s. An internal audit concluded that the committee papers were destroyed during upheaval precipitated by a Functions and Manpower Review which probably resulted in either a delegation of responsibility without proper instruction or an assumption of responsibility without proper authorisation. Audit is annexed. Zubeda Seedat advises Sir Nigel not to meet with Lord Jenkin but instead to send a letter (attached) explaining in detail the departments understanding of why papers were destroyed.
