Guidance on how to request medical records
The Inquiry recognises that accessing your medical records is not always straightforward and has therefore provided guidance to help you in doing this. This guidance also covers how to access someone else's medical records (with their consent) and how to access the medical records of a deceased family member.
It is not essential to have a copy of your medical records to provide evidence to the Inquiry, but your records may strengthen what you want to tell the Inquiry.
